Any medical professionals out there? Physical therapist or chiropractor or sports medicine or....

Kind of a long shot but I'm at a loss.

Story: I'm 41, healthy, no real history of injury. I fell off a skateboard! According to the chiropractor, I sprained my SI joint and jammed my hip joint.

SI joint feels fine now - about a month later. Hip joint hurts daily. Pain level about 3 or 4 out of 10. Not bad but noticeable. My pedal stroke is challenged as well. I've also noticed running is more strained. It's like my leg got shorter.

I've tried stretching, traction, foam rolling and another chiropractor - he did traction on it as well, but to no avail.

My question is what specialty do I need to go see? Physical therapist or chiropractor (which hasn't helped much) or sports medicine or....what else is out there. Which specialty should I see?

My concern is that it will heal this way and later on I'll need surgery to fix it. I'm hoping that STers with actual medical knowledge will jump in here.

See somebody with an MD. Chiro is a waste of money at best.

Sports med, orthopoedist... Etc.

ETA: not a doctor Just someone with a long history of sports injuries.

Go see an orthopedic surgeon who specializes in hips or one with a sports medicine fellowship.

Almost every chiropractor finds an SI joint issue that needs to be "adjusted/out of alignment" when it comes to lower extremity issues. SI joint is poorly understood and most of the time not the cause of one's symptoms.

I agree with the above. Go see a sports med/ortho doctor. It's almost always a good idea to follow up with a real specialist when the pain persists after a traumatic injury like you had.

It's interesting you say that because I felt like I kept saying my back doesn't hurt anymore my hip is what hurts. But, the treatment was still heat and stimulation on my back with an adjustment following. The second chiropractor didn't do heat/stimulation but he would lightly pull my leg - traction I guess - and then crack my back. Either way, the hip was almost always avoided.

Heat/ice/stim/ultrasound combos are all treatment of choice from 1980s. Real practitioners know that modalities are rarely indicated, especially for someone with an athletic background. It's just a money grab. You go in with pain, they appliy stim and heat to mask the pain (gate control therapy), distract the hip joint (not actually treating the underlying problem but feels good anyways) and you leave the clinic, thinking they did something useful. But, you need to be consistent with coming back otherwise it doesn't work!

Any chiropractor/physical therapist who gives you the stim/heat/ultrasound combo is a crook and is wasting your time. You might as well buy yourself a stim unit on Amazon for $30.

If “jamming your hip” resulted in a bone bruise or fracture of some sort that’s going to take more than a month to heal.

Where are you located? If you are anywhere near academic sports medicine, I probably know someone. Or can warn you off of some folks.

See a doctor, get some scans done.
All for PT when indicated, but given how long this has been going on for, you would be best to see an MD, get some decent imaging, get a proper diagnosis (not one just made on clinical acumen alone), then get treatment appropriate to that diagnosis.

Physio here. It would be a bitch to give you any kind of online/forum advise as we can't do an assessment this way. See a doc, do a scan...what others have suggested.

I am a chiropractor with 35 yrs experience and I've seen hundreds of people with your symptoms and treated with a reasonably high success rate. The problem is that nobody can make a diagnosis over the internet. My suggestion is that if it is this stubborn you might want to get a MRI as this will give far more info than just a regular x-ray and then take it from there.

Whenever I see people asking for medical help in online forums, I think that it's the reason that chiropractors still exist.

I agree about the diagnosis over the internet. I wouldn't ask for one nor would I accept the diagnosis over the internet.

I just wanted to be clear that what I reached out for was a recommendation on which area of medicine/practice to see - not an actual diagnosis.

Once the choice in specialty is made I'll let that person make the diagnosis :-)

Shows how very, very little you know. Chiros don't make any money on MRI's. In fact where I practice we can only order x-rays but are supposedly getting new legislation for ordering CAT scans from hospitals or private radiology labs. We can't order MRI's but we can always request the reports which I do often. They're covered by our government health insurance in Ontario but MRI's can only be ordered by family physicians or specialists. Whenever I've thought it might be beneficial to get an MRI I've sent a note to family physician and have never been turned down. MRI's can give a lot of information that you won't get with other procedures. I wouldn't recommend them as routine but in cases that aren't responding as expected it doesn't hurt to dig a little deeper.

Bob Dimeff is in Dallas. Friend of mine. Good guy with good reputation, and takes care of the Dallas Stars. Past-president of our main sports medicine society (AMSSM).

He just left his academic practice at UT Southwestern and joined a private group called Texas Orthopedic Associates.

definitely start with a sports orthopod. My 2 cent guess is that you'll get a MRI of the hip which will show a bone bruise or torn labrum in the hip. But the internet diagnosis is worth less than 2 cents.

He said an MRI, not a weird device that no one has ever heard of that looks into your soul. MRI would be standard of care at this point by anyone who knows what they're talking about.

To the point of this thread, there are MDs and DCs who absolutely know what they're doing and what they're talking about. BUT there are many many MDs and DCs, PTs, whatever, that shouldn't even have a license let alone be qualified to take on this case.
