Touchy subject

Have a client that when her training goes up she starts to develop a U.T.I. from the mileage increase. Her shorts are kept well and cleaned after each ride as well as having multiple shorts. We have tried two different chammy creams as welll as no chammy creams for at least a month stint and with no luck. I hate not helping to my fullest ability so I am turning to one of the best tri forums to ask for assistance.

hmm, is she being sure to shower right after her ride, not staying in the shorts any longer than necessary?

Have not gone to a physician as of yet, but The first thing that I had suggested is not to hang out in her kit if at all possible. She does shower right after now and with the miles starting to pour on I do not want this to keep her in the uncomfortable area as to where she might hold back any. I will tell her to talk to a obgyn and see if there is something that I may be missing.

in addition to that - cranberry extract. Buy at health food store. Far more concentrated than juice.

What is she doing about bathroom breaks on long rides? Holding it in to stay on the bike could contribute to UTI. Also, not sure OB/GYN is the correct type of physician for her to see.

My $.02 is to try SportSlick. It's an anti-bacterial, anti-fungal chammois lubricant. As it is clear, it also doesn't stain shorts and it doesn't sting, like products with witch hazel.

In my experience, more fluids may help her situation.

Is she hydrated properly? Not just during her training rides, but all the time. Be sure she is drinking enough water and going to the bathroom frequently. Also, maybe add some cranberry juice - not the juice coctail, that has too much sugar and not enough cranberry juice, but pure cranberry juice. It's strong and quite tart, but drinking an ounce or 2 a few times a day may help.

Has not gone to a physician yet? Bad plan. Recurrent UTIs are often an indicator of kidney problems. She needs to start with her GP and move to a specialist from there.

Yes. Is she wearing thick padded pants? It is possible to contaminate the triangle because bacterias get trapped in the padding, especially those that cause yeast infections.

Hydration and vitamin C are key to prevention. I've been battling UTIs since I was a kid. When I was in my late 20s a doctor mentioned the vitamin C thing to me. It has changed my life. For me, it works far better than cranberry for prevention.

It is not uncommon for women to get UTI's after sex. Likely the same mechanism here (friction, short urethra). She should see her general MD. If they are satisfied that is all it is, she can just take prophylactic abx after long rides.

This is actually Very frequent advice from ER docs to urologists for folks with frequent UTIs.
