Can't fully extend arm after bike accident

Took a spill a few weeks back - hit some loose cravel making a turn and bike went out from under me. Ended up landing on the right side of my body - mainly upper body. Ended up having a stress fracture in my shoulder blade - out 4-6 weeks. Doc also xrayed my forearm/elbow and there were no fractures/breaks. They could only xray the outer part of my forearm/elbow as the pain was so bad i couldnt do anything but keep my arm bent across my stomach. Saw an sports orthopedic surgeon and he agreed stress fracture was only issue. It's been 3.5 weeks since the accident and I am still in a sling - per doc guidelines. But I do take it out for an hour or two each day just to let the arm hang to the side. I still keep the arm close to my body - per doc orders. But I wanted to get the arm moving below the elbow so it doesn't stiffen up.

Even after a couple hours I still can't put my arm completely straight down. When my palms are to my side - my arm goes mostly straight. When the palms face out in front of my body I can only get it down about 90-95% before it just stops. And then if i were to twist my palm from facing out in front of my body to facing outside of my body - you can see a gap where my elbow bends away from my body. Where as my left side (non-injured) touches my body.

I go back to the Ortho Surgeon next week, but wanted to see if anyone had any thoughts on what the issue might be. I dont know if i broke something on the inside of my elbow that's causing the issue or if it's tendon related and/or just bruised/inflamed still. I had some swelling for about a week or so in the elbow, but a lot of my arm was swollen/internally bruised. Hoping it's just bruised. I have the HIM WCs in 2.5 weeks - clearly not going to 'race-race' it, but want to complete it. The HIM WCs have a little more meaning to me than just a race - it's been a long journey to make it back.

Any help/thoughts would be great!

Many radial head fractures (in the elbow) don't show up on initial X-rays. They tend to limit range of motion as you describe.

That's my guess as well. Crashed on my mtn bike and had the same symptom of not being able to straighten my arm. Turned out to be a radial head fracture.

How long did it take to heal? Need surgery ir just stay in a sling for a fwe weeks? Been in a sling for 3 weeks.

It's gonna take a fair bit of physiotherapy. I dislocated my elbow two years ago and was in a range-restricting brace for a while, and same deal: super locked up once the brace was off. Part of the problem was with my dislocation, the surrounding muscles really tightened up to protect the area.... and stayed tight. The palmaris longus (medial muscle of the forearm) got super tight. Visits to the therapist every few day consisted of a parrafin dip of the elbow to relax the muscles followed by massage from the PT then passive weighed stretching (light sandbags) while laying prone on a table. Then active stretches with a dowel, unweighted triceps pushdowns and massage. My wife's a trained PT and did a lot of work on me when insurance was used up. I cursed her, but she said if I don't suffer the pain of stretching the upper forearm muscles, I wouldn't get the range back I wanted.

I still don't have the full hypermobility I had before the dislocation, but I have enough to swim, ride, etc. without any hitch in my giddy-up. It's just a slow process.. but get on it if your doc says you can start. Only he/she will know if you still have internal healing to go, a bone chip floating around in there (which I had for a bit), etc.

3.5 weeks since accident?
Broken radial head takes 6 weeks to heal.
When I broke mine my body put a total lockdown on that elbow for the entire 6 weeks- I could not even hold a cup of coffee with that hand. Could not use my arm to push myself up off of my stretching mat.
Zero strength, limited range of motion. Hurt like hell if I tried to use it.
Started improving rapidly after 6 weeks.

yea accident happened 7.26. it doesnt hurt that bad. it hurts here and there but nthing that bad. its close to bein able to fully extend but missing few degrees of extension

Get out of that sling as soon as possible and start straightening that elbow as much as possible.
It will hurt but it will heal.

Depends how badly you break it. I broke both mine (2 cyclists in front of me went down, I hit the brakes, flew over the top and landed on my hands), doc told me to get them moving again ASAP so I didn't lose the movement. I was only in slings for 2 days, was having physiotherapy within a week of the accident, and was training again within a month.

To the OP - as always, YMMV, seek medical advice but if at all possible do so from somebody who is used to working with athletes.

I was just under the measurement where they recommend surgery. I guess sideof the radial head was depressed causing a fracture down the middle. Surgery would be to remove the radial head and re-install a new shiny one. I was in a sling for 5 weeks. I had more issue with my shoulder getting stiff than my elbow. Doc said I woudn't have anymore hyperextension in my arm but I actually have a little.
