Shopping for my first road bike after riding a mountain bike for years. the LBS said, "take it for test ride!" so I did. While riding around the neighborhood I failed to unclip at an intersection. hurt my pride far more than the bike. So I rode straight back to the shop and said I'll take it.

I was reminded by a buddy last night that my driveway failure was karma from an incident in 1994.
The fall semester of college was starting in a few days. Our fraternity house had a sand volleyball court in the front yard. We would pretty much drink beer and play volleyball all day (with an occasional class thrown in to break it up a little).
The yard was pretty full of guys and girls.
This kid comes rolling up on his brand new mountain bike/classroom cruiser (with cages/straps on the pedals). He was friends with one of the guys in our house and was going to "Rush" our fraternity that semester.
As he pulls onto the sidewalk of our house with a big smile on his face, he realizes that he is stuck in the pedals. He falls over. I immediately run over to him and point and laugh and say "Who is the dumbass?" while the yard full of people laughed at him. (Yep, I was the douchey frat guy. Sorry).
He jumped up as fast as he could and acted like nothing happened. I think he redeemed himself by shotgunning a beer in the next few minutes.
He and I are best of friends to this day.
I guess we are even now.

I was adjusting the brakes slightly. I rolled down my driveway, came to a stop and realized that I needed to unclip as I was falling to the non-drive side.
Last year, I was out on a ride and had just finished discussing failing to unclip when I came up to the next traffic light. I unclipped drive side and managed to fall over to non drive side - about a foot away from a car.
Most of my falls have resulted in drive side falls, strangely resulting in nailing the back of my leg against the chainrings.
I'm competing against my current fitness level, and planning to kick my a**!

I was trying to impress this girl I was riding with and came to 4 way stop. I tight I'd do the cool thing and try to balance the bike by standing off the seat and turning the front wheel back and forth. While the cars were going by and everything I couldn't clip out in time and went straight over on my side

Ok, how do you forget to unclip? With less than 3 seconds between stopping and needing to take the foot out even a goldfish would be ok!
To the OP, I think I am the only person that I know who has been riding on clipless pedals for more than a couple of years and hasn't gone down by this method. Every red light or stop sign I still approach thinking "this will be the one." Eventually I will be right!

My most embarrassing one was with toe clips 20 years ago in San Jose. Rode up to a Subway with a friend to fill water bottles, I pull up next to the door, and after I stopped, I remember I need to loosen the strap. Balancing I try to reach for the foot, and of course, don't quite make it. Over I go onto the grass.
Then I look up, and there's a dozen people having lunch, enjoying the spectacle.
Another tip over was totally not my fault....
I lent a friend my old road bike, and we went on a group ride to Back Bay (in Orange County, Calif). I stopped with the group, and waited for the stragglers, which included my buddy. He came up beside me, and proceeded to tip over, into me, and I went over, into a pile of horse manure.

I'm a rightie that unclips on the left. I don't really get off the seat when I stop (I'll learn one day) and I lean left naturally. Tried to unclip right just for "safety" and I fell to the left because even though I unclipped right I still leaned left. So now I unclip and lean left happy.

I un clip on the nds because if I un clip on the drive side I end up with a chain ring tattoo on my calf!

I did this after an awards ceremony when I was the overall winner. Short ride from the park at the finish to my hotel. Lots of people leaving the race site to see. I was proud to be an example of the grace and athleticism that defines out sport. "There goes our race winner..." Plop.

Had friend that wrecked in his driveway and fractured his hip yesterday. He is in his 70's and was returning from a ride. Experienced rider, but hit some gravel. Hip replacement/fx surgery today.....

My wife had a similar fall about six weeks ago. She broke her fall with her arm causing a radial head fracture and a hairline fracture in the wrist. Don't be surprised if you see a classified ad for lightly used Shimano 105 pedals very soon.

I crashed when I rode clipless for the first time, many many moons ago. Came to a stop near a busy intersection, forgot about unclipping, and tipped over on my side in full view of rush hour traffic. Weird though.... that was about 10 miles into the ride. I managed to remember to unclip several times before I fell.
I've had a few close calls over the years, with new cleats or new pedals. One time I couldn't unclip because shoes hit the frame when I twisted to unclip, before the cleat disengaged. I don't panic anymore. Just pedal more if you can't unclip. Or look for something to grab onto.

Rode Lawrence 70.3 with masher pedals on my road bike and now I'm trying to switch to clipless... I can't decide if it makes me more confident to hear that even you guys still wipe out or if it terrifies me even more.

I wouldn't worry about it. Most of what you have read above only happens at slow/almost stopped speed and not many serious injuries.
If you get hit by a car/another cyclist, I don't think it mattes what kind of pedals you are using.

Went down on my driveway today, before I even started my ride. Quick check to make sure the neighbors didn't see and I was on my way. No worse way to start a ride but I think it happens to everyone at least one...maybe some (like me) more regularly lol

When you make the move to clipless, just accept that you *will* fall over. It is not a matter of if, it is a matter of when. Accept it, and accept that all that will really be hurt is your pride.
After last nites MTB ride, I'm thinking about making the switch there. Trees, here I come...

When I got my first set of pedals with straps I promptly cinched them down and hammered out of the driveway up the street. As I came upon the first stop sign, absolutely no part of my brain considered the fact that I should loosen the footstraps. It was only as I was falling over on my side that it occurred to me. I was a decently smart kid with good foresight so I still can't understand how I never considered how I was going to put my feet down.
