aerobar cable routing

Hi there

So I get to borrow an integrated aerobar/cowhorn for my next TT. I'm fighting the cables trying to set up Ergopowers on the cowhorns and having the cables go as aerodynamically as possible. However, the cables emerge from the Ergo unit pointing "up", and the entire bar is "down", if I try doing 180 in such a small space, I found shifting to be terrible. How is it done?
P.S. I don't mind having perhaps the rear derr cable stay "out", but at least the brake cables should be hidden, and I've seen numerous times ( on prologues, technical courses, etc ) where the rider opts not to use bar-end shifters.
