Triathlon injury triage & prevention

Over my first couple of seasons I've had IT band issues & a small glute medius tear, currently the inside back of my knee isn't happy (pretty sure it's hamstring related).

All of these seem to be fairly standard triathlete issues. By finding the appropriate solution (foam rolling, stretches, monster walks etc...) they normally clear up.

Problem is finding the solution not only takes time, but requires an injury to highlight it first. so:

1) Do you know of any online service's for sports/triathlon injury triage?
2) What do you do to avoid injury / manage old ones

Our IREP Athletics Coaches also do Functional Movement Screening and create a develop a custom corrective exercise regimes for athletes to mitigate risk of injury.
