Shoes: No improvement over specialized trivent for me: might be dependent on pedaling style as someone I have tested at the tunnel actually did save Watts. But I checked twice, no gain for me. I raced with the trivents, as the toebox is so narrow in those s-works that I can only stand them for 90 min of riding.
Top tube bottle: I cannot reach bottles behind me, but wanted another bottle. Small aeropenalty for me that I gladly take. Unfortunately the setup started to come loose during the race.
Oversized pulleys: No idea. Did not test with regular pulleys.

Thanks!

BTW for that position do you recommend a two beat or six beat kick?

You need to set the cranks at 90/270 for best results. Save another 1 watt.

In all seriousness, could you actually get into that position while riding?

Yes easily done on a road bike. But I will leave it to someone else to try that on a TT bike in the aerobars.

Every season during a slow point in the early season races, some pro will do it to get a little camera time for sponsors and break up the monotony.

Serious question: which bottle cage do you have along your top tube? I've contemplated a similar setup.

Yeah, but at what yaw? Also pedals parallel to the ground will increase the watt savings over 121 miles significantly.

It was the ''worlds fastest motorcycle'' well into the modern era. The Vincent Black Lightning even has a song written about it;
