Most economical packing and shipping method for a bike

What have you found to be the most economical way to ship a bike in the CONUS? I have sold a 48cm frame bike without wheels. Who have you used to ship and how have you packed the bike? Is getting the LBS to box it up the best method for packaging?

~$45 for a normal size bike box includes pick up by FedEx. Without wheels the box could be smaller and will be cheaper. Insurance is $10 per $1k of insurance.

Get the box from your LBS but you can pack it yourself. Buy foam piping insulation and bubble wrap and pack carefully
