Lack of progressive overload -- stagnation

I don't follow a specific plan perfectly -- I have 2 faster mid length runs on Tues/Thurs, a longer run on Saturday, and one or two short easy runs during the week. I do intervals on the bike on wed/fri and a longer, steady ride on sunday. I swim 4-5 times a week.

I don't necessarily adjust my volume/intensity too often (other than FTP improvements), but am I doing myself a disservice? I feel like absolute crap today (with my A race in a week being St Anthony's) and I'm pretty irritated by it. I just spent a good portion of my afternoon on Golden Cheetah looking at the PMC chart. Instead of the sawtooth increase oyu *should* see, my CTL has essentially flattened...

Is it reasonable to believe that I will get much faster by just doing the same old runs in and out? Or do I really have to increase mileage 10% every week?

Any n=1 experience is always appreciated.
