How could this happen to my BB? Pics!

Noticed a clicking sound coming from my crank/BB area so took it apart to find this. Scratching my head trying to figure out how this piece of the bottom bracket on the non-drive side could just be gone. Any ideas? It's only a year or so old. Can it be fixed? Assuming not, anyone have a GXP Ceramic BB they're looking to part with?

Advice: drop the ceramic bottom bracket. There are way better alternatives that compete strongly with ceramic and last much longer.....

And that just looks like a bracket that was left in too long or potentially axially compressed too much. The seal looks really worn, making me think something happened during installation

most notably hawk racing. Tested well in friction facts test. Pricey for a BB compared to standard sram/shimano threaded but they are butter. 0.32 watt consumption is darn low.
