I was hoping to get some suggestions from ya'll. I was fitting a co-worker's cleats. He said he feels like his left foot is falling off the pedal on the outside. I moved his foot out and wedged it, but he says he still feels like his foot is falling off. I was thinking of putting on a pedal extender. Any other thoughts/suggestions?

A few more 'eliminate the possibilities' diagnostics to isolate what might be causing the issue.
does he have this same sensation in a different brand of shoe
does he have the same sensation in same shoe, different pedal
check that the pedal spindle isn't bent

Was the first wedge installed in a "varus" position (thick side-inside)? Try installing it in a "valgus" position (thick side-outside).

This is a fairly common complaint. I hear it at least once every two weeks. The solution is almost always the same.
First - Stance width and wedging are generally two different things. They certain can relate but there needs to be another reason for adjusting the stance width.

Well, valgus is one-in-a-million, and wedging and stance width are two different things.
The OP doesn't say how many wedges or what pedal system, but if we eliminate other possibilities such as those I wrote earlier, then you should wedge more. Stacking 4-5 under the cleat isn't uncommon, plus adding 2-3 inside if necessary (as long as it doesn't choke up the fit of the shoe). My personal record for wedging alone was six on the outside and three on the inside. Uffdah! More wedges means longer screws....
I have had the experience of not wedging enough, being too conservative, and so don't provide good solution for rider.
I do recommend the online class Swifty is promoting, btw.
