Does this come with the BB and chain rings, or just the crank arms/spider? How old and how many miles?

Chain rings, Yes. BB, I can include a GXP with very few miles for an extra $15, standard 68mm threaded.

The quarq itself? not sure how old. It's not a current model, but still runs very well.
