No idea for sure, what jumps to mind is trying to remove an old or failed femoral joint side of an artificial knee.

In a nutshell, if I remember correctly*, the SawsAll got stuck in the miter box, or the drill bit was a little too big for the pilot hole

Been back home now for about two hours. Having some pain & discomfort - trying to gauge level compared to when I had the other knee scoped 13 years ago.
