I'm not sure if they do bottles, but I used Northwest Territorial Mint for challenge coins. I know they do some other stuff as well, so you might find something there. They were slightly more expensive than some options, but I got really high quality work and good customer service from them.

If you're not on a tight budget Polar does custom bottles. They are made in the USA. I got two when I bought my VW that held up better than the car did :-)
