I've been using Umlet recently.
Dia is also good but not sure how well it runs on Windows.

Visio. You can use word but it can be cumbersome.

Agreed with everyone. Visio is the best I've seen. Not certain about any good, reliable free ones though.

Used many, for simple stuff I sometimes use PowerPoint. It merges better with bosses skill sets and you can animate the diagrams.
