I ride with my index fingers on the shifter pivot cover, so my thumbs are about in the center of the shifter lever, if I rolled them up there. Generally my thumbs are on the inside of the levers hanging in the air.

Thumb on the inside, index finger over the shifter with the big knuckle just behind the leading edge. Enough force to move the lever with my index finger, alone.

Hands wrapped around them, so you basically can't see them.

Thumb on lever, rest of hand curled under extension, middle finger rests on shifter bolt.

palms behind on the ski bend
Index fingertip touches the buttons- which I rotated 180 degrees from their standard position to be on the 'outside' face of the aerobar stalks.

for the horn position, my thumbs hit the buttons

Base position is thumbs on top and fingers curled around under so they're mostly in my hands and ready to shift either way, with maybe the tips sticking out.

Variation 1 is cupping my hands inward more with the levers sticking out between my index and middle fingers, just kind of on a whim if I'm in a stretch with little or no shifting and trying to focus on relaxing and not gripping the bars too hard;

Variation 2 is my thumbs curled more to the inside and under with my palms down and fingers forward on top of the levers if I'm maybe trying to squeeze a bit longer and lower (visualizing "flying" position) for a flat segment into an extra-stiff headwind.

Sometimes on a small riser I'll scoot back slightly on the saddle and choke up on the S-bends, so then of course I gotta reach to shift, but I'm already a bit more upright so it's no biggie.
